Service Coordination
Service Coordinators are assigned to all consumers actively receiving any service of the GCDSNB, with the exception of Intermediate Care Facilities. Responsibilities include intake, eligibility determination, development and implementation of a single plan to address the service needs of each consumer, monitoring of services, referrals for needed services, advocacy, choice of service, measure of satisfaction, and collaboration with all professionals.
Medicaid Waivers
Through Service Coordination, the Agency implements two Medicaid Waiver programs, the MR/RD (Mental Retardation / Related Disabilities) Waiver and the HASCI (Head and Spinal Cord Injuries) Waiver. These programs are available to consumers who meet intermediate or skilled level of care, but are being served in their homes and have additional Medicaid services made available to them such as: personal care aides, additional prescription drugs, environmental and vehicle modifications, medical equipment, supplies, and assistive technology services.
Respite Care
This is a temporary break in caregiving responsibilities for family members of consumers residing at home. It is delivered in several ways: an Agency trained sitter can be sent to the consumer’s home; the consumer may spend time in the home of a licensed respite provider; the family can choose its own caregiver and be reimbursed for the cost at established rates; or the consumer can receive Respite in a licensed Respite facility.
Companion Care
Companions are Agency trained caregivers assigned to adult consumers. Companions provide training in activities of daily living, to consumers residing at home, to increase their independence.
Attendant Care
Attendants are Agency trained caregivers who provide assistance to HASCI consumers in their homes. They are supervised by a registered nurse.
KidVentures
KidVentures
is the early intervention program of the Greenville County Disabilities
and Special Needs Board. This service focuses on families of children
with special needs from the ages of birth to six years. Using the
natural environment of the home, the Early Interventionists work with
the child one-on-one and teach the family how to work with the child
to develop and/or improve motor skills, social skills, cognitive and
self-help skills/development, and independence. We also provide Service
Coordination for these children, assisting the family to utilize community
resources.
